VII. Required Documents

The applicant will be directed as to when documents are to be submitted.

1.  A letter from the inquirer’s pastor nominating him to St. Francis School of Theology for deacons.
     If the inquirer is married a letter from their pastor introducing the inquirer’s wife and attesting to
     her practice of her faith.
2.  A complete application form, and if married an application form from the wife of the applicant.
3.  An autobiography that will focus on particular areas, and if married an autobiography that also addresses
     particular areas.
4.  Consent for a background check.
5.  Consent for psychological testing.
6.  A letter from the wife of a married inquirer giving her consent for her husband to apply for and if accepted
     to enter into the Inquiry period.
7.  Church certificate(s) of Baptism, Confirmation, and, if relevant, marriage, issued within the past six months.
8.  Proof of age.
9.  Letters of recommendation.
10. A recent medical report.
11. Official transcripts of past or present academic studies.
12. Proof of legal residency in the diocese.
13. A letter of recommendation from the applicant’s employer.
